Over the past decade the Commission has published a certain number of recommendations covering protection from radioactivity and safety in radiation based medical applications. This report (Publication CIPR 105) updates the 1996 Publication n° 73. It is one of the "foundation documents" which underlie the 2007 CIPR recommendations. The recommendations it contains are especially important considering the fact that medical exposure represents 95% of exposure to radiation resulting from human activities. It addresses professional readers, practicians and associated medical staff involved in medical applications of radiation, who are seeking detailed information on  protection policies for medical exposure.

The International Commission on Radiological Protection (ICRP) regularly publishes general recommendations that make up one of the main international reference documents in the field of radiation protection. The latest edition, Publication 103, was published at the end of 2007 (ICRP 103), replacing Publication 60 (ICRP 60), which appeared in 1991. The new recommendations were adopted at the end of an open process lasting nine years. Meetings, seminars, conferences and web consultations took place throughout discussions with professionals.

Environmental protection gives rise nowadays to frequently lively debate centring on fears surrounding the existence of zones polluted by previous activities. National and international protection policies are being developed to respond to these questions. These policies call on scientific, yet fragmented, knowledge on the effects of radiation on the environment. In the field of radiological risk, where a purely medical approach focusing on human health has prevailed, a paradigm has emerged stipulating that "protect man and you protect the environment". Specialists in radiological protection are challenging this paradigm. ICRP suggests a methodological framework to protect fauna and flora against ionising radiation.

The International Commission on Radiological Protection (ICRP) was founded in 1928 upon the request of the radiologists. It provides recommendations concerning all situations which human being may encounter when exposed to ionising radiation from natural or artificial sources. The purpose of this publication, translated in French, is to support the prevention of accidents involving patients undergoing external radiation therapy or brachytherapy. It does not directly deal with therapies using unsealed sources. It is intended for a readership of various professionals involved in the radiation therapy procedures, hospital administrators, Public Health authorities and regulatory agencies. The approach adopted is the description of exemplary severe accidents, the study of the causes of these events and the contributory factors, the summary of their disastrous consequences and the proposal of recommendations for their prevention. The measures suggested include institutional measures, staff training, quality insurance programs, follow-up adequacy, clear definition of responsibilities and rapidity to report events.

Interventional radiology has drastically changed certain surgical procedures by giving them massively increased efficiency and precision compared with previous practices: the benefit is considerable for patients. However, this technique presents risks related to ionising radiation exposure, for both operators and patients: these risks must be assessed and then minimized. How can radiation injuries caused by these practices be avoided? The ICPR has written the present report to answer this question and to provide doctors and operators with recommendations concerning the best radiological practices that should accompany intervention procedures.
